การพัฒนาระบบบันทึกทางการพยาบาลของผู้ป่วยโรคเรื้อนสถาบันราชประชาสมาสัย

Abstract

การวิจัยครั้งนี้เป็นการวิจัยและพัฒนา มีวัตถุประสงค์ (1) เพื่อพัฒนาระบบบันทึกทางการพยาบาทของผู้ป่วยโรคเรื้อน สถาบันราชประชาสมาสัย และ (2) ศึกษาผลของการใช้ระบบบันทึกทางการพยาบาลในด้านคุณภาพของการบันทึกทางการพยาบาลและความพึงพอใจของพยาบาลต่อระบบบันทึกทางการพยาบาลก่อนและหลังการพัฒนา กลุ่มตัวอย่างแบ่งเป็น 2 กลุ่ม ซึ่งได้จากการสุ่มอย่างเจาะจงได้แก่ (1) พยาบาลวิชาชีพที่ปฏิบัติงานในหอผู้ป่วยโรคเรื้อนอายุรกรรมและศัลยกรรม จำนวน 15 คน และ (2) แบบบันทึกทางการพยาบาลของผู้ป่วย จำนวน 60 แฟ้ม เครื่องมือที่ใช้ในการวิจัย คือ (1) ระบบบันทึกทางการพยาบาลที่พัฒนาขึ้น (2) โครงการอบรมพยาบาลวิชาชีพและแบบทดสอบความรู้เรื่องการบันทึกทางการพยาบาล และ (3) เครื่องมือที่ใช้เก็บรวบรวม ข้อมูลมี 2 ชุด ได้แก่ แบบตรวจสอบคุณภาพของการบันทึกทางการพยาบาล และแบบสอบถามความพึงพอใจของพยาบาลต่อระบบบันทึกทางการพยาบาล เครื่องมือวิจัยได้ผ่านการหาความตรงตามเนี้อหาและความเที่ยงค่าความเที่ยงของแบบตรวจสอบคุณภาพของการบันทึกทางการพยาบาลแบ่งเป็น 3 ตอน ได้แก่ ความครอบคลุมตามกระบวนการพยาบาล ความถูกต้องตามหลักการบันทึกและความต่อเนี่องของการบันทึกเท่ากับ 0.95, 0.85 และ 0.94 ตามลำดับ และความเที่ยงของแบบสอบถามความพึงพอใจของพยาบาลต่อระบบบันทึกทางการพยาบาล เท่ากับ 0.92 วิเคราะห์ข้อมูลโดยการหาความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน Mann-Whitney test และ สถิติทดสอบที ผลการวิจัยสรุปได้ดังนี้ (1) ระบบบันทึกทางการพยาบาลของผู้ป่วยโรคเรื้อน สถาบันราชประชาสมาสัย ประกอบด้วย วิธีการบันทึกแบบบันทึกและคู่มือการใช้ระบบบันทึกทางการพยาบาลตามกระบวนการพยาบาล ชึ่งทำให้บันทึกได้อย่างรวดเร็ว ครอบคลุมความต้องการของผู้ป่วยแบบองค์รวมและอย่างต่อเนื่อง (2) ค่าเฉลี่ยของคะแนนคุณภาพของการบันทึกทางการพยาบาลและความพึงพอใจของพยาบาลต่อระบบบันทึกทางการพยาบาลโดยรวมหลังการพัฒนาสูงกว่าก่อนการพัฒนาระบบบันทึกทางการพยาบาลอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01

The purposes of this research and development were: (1) to develop a nursing record system for leprosy patients at Raj Pracha Samasai Institute, and (2) to study the outcomes of using the record system in terms of quality of nursing records and nurses’ satisfaction with the record system before and after the development. The samples were selected by purposive sampling including 2 groups: (1) fifteen professional nurses in medical - surgical wards, and (2) sixty nursing records for patients. Research tools consisted of (1) the developed nursing record system, (2) a training project and a knowledge test of nursing records, and (3) two types of data collection tools (a) a quality of nursing records audit installment and (b) the nurses’ satisfaction with the nursing record system. All tools were tested for validity and reliability. The reliabilities of the quality of nursing records which were divided into three sections (a comprehensiveness of nursing process, an accuracy of charting, and continuity of charting) were 0.95,0.85, and 0.94 respectively. The reliability of nurses’ satisfaction with the record system was 0.92. Data were analyzed by frequency, percentage, mean, standard deviation, Mann-Whitney บ test, and t-test. The major findings were as follows. (1) The nursing record system for leprosy patients included record procedures, forms, and a manual which were constructed according to a nursing process. The system contributed to quick record, holistic approach for patients’ needs, and continuity of charting. (2) The mean scores of the quality of nursing record system and the nurses’ satisfaction with the record system after the development were significantly higher than before (p <0.01).
