Abstract
การวิจัยครั้งนี้เป็นการวิจัยและพัฒนามีวัตถุประสงค์เพื่อ (1) วิเคราะห์สภาพการณ์และความต้องการ การจัดการรายกรณผู้ป่วยมะเร็งเต้านมที่ได้รับการรักษาด้วยการผ่าตัด (2) สร้างรูปแบบ และ (3) ประเมินความเหมาะสมของรูปแบบ ที่พัฒนาขึ้นไปใช้ กลุ่มตัวอย่างมี 2 กลุ่ม กลุ่มแรกเพื่อวิเคราะห์สภาพการณ์และความต้องการการจัดการรายกรณีของโรงพยาบาลนี้ ได้รับการคัดเลือกแบบเฉพาะเจาะจง จำนวน 18 ราย ได้แก่ ศัลยแพทย์หัวหน้ากลุ่มงานศัลยกรรม 1 ราย ศัลยแพทย์ 1 ราย ผู้ช่วยหัวหน้าพยาบาล 1 ราย หัวหน้าหอผู้ป่วย 2 ราย พยาบาลวิชาชีพ 7 ราย ผู้ป่วยมะเร็งเต้านมที่ได้รับการรักษาด้วยการผ่าตัด 5 ราย และสามีผู้ป่วย 1 ราย กลุ่มตัวอย่างที่ 2 เพื่อประเมินความเหมาะสมของรูปแบบ จำนวน 11 คน เป็นกลุ่มตัวอย่างเดิมเดิมและเพิ่มอาจารย์พยาบาล 1 คน เครื่องมือในการวิจัยมี 2 ชุด (1) ประเด็นสนทนาเพื่อวิเคราะห์สภาพการณ์และความต้องการในกาวจัดการรายกรณีผู้ป่วยมะเร็งเต้านมที่ได้รับการรักษาด้วยการผ่าตัดสำหรับผู้ให้บริการและผู้รับบริการ (2) แบบประเมินความเหมาะสมของรูปแบบที่สร้างขึ้น เครื่องมือผ่านการตรวจสอบความตรงตามเนี้อหาจากผู้ทรงคุณวุฒิ 4ท่าน ค่าดัชนีความตรงตามเนี้อหาของเครื่องมือทั้ง 2 ชุด เท่ากับ 0.88 และ 0.83 ตามลำดับ เก็บรวบรวมข้อมูลโดยการสนทนารายบุคคล และกลุ่มด้วยสุนทรียสนทนาวิเคราะห์ข้อมูลเชิงเนี้อหา และสำนวฌค่าคะแนนความเหมาะสมผลการวิจัยพบว่า 1. องค์ประกอบของรูปแบบประกอบด้วย 1) ด้านโครงสร้าง มีการกำหนดนโยบาย เป้าหมายและการมอบหมายงานที่ชัดเจน มีมาตรฐานการปฏิบัติ 2) ด้านกระบวนการในการนำรูปแบบการจัดการรายกรณีสู่การปฏิบัติและการติดตามประเมินผลอย่างต่อเนื่อง 3) ด้านผลลัพธ์ผู้รับบริการ ได้รับบริการที่มีคุณภาพตอบสนองความต้องการ และมีความพึงพอใจต่อบริการ 2. ประเมินความเหมาะสมของรูปแบบการจัดการรายกรณีผู้ป่วยมะเร็งเต้านมที่ได้รับการรักษาด้วยการผ่าตัดในโรงพยาบาลแห่งนี้ พบว่ามีความเหมาะสมในการนำไปใชัโดยมีค่าเฉลี่ย ร้อยละ 91.16 มิติ ด้านความเป็นอิสระของการพัฒนารูปแบบ มีคะแนนการประเมินความเหมาะสมมากที่สุด คือ ร้อยละ 96.88 มิติ ด้ามความชัดเจนและการนำเสนอ มีคะแนนประเมินความเหมาะสมน้อยที่สุด คือ ร้อยละ 88.02 ข้อเสนอแนะสำหรับโรงพยาบาลที่ต้องการนำรูปแบบการที่พัฒนาขึ้นนี้ ไปใช้ จำเป็นต้องพิจารณาถึงความต้องการของบุคลากรและความเหมาะสมกับบริบทของโรงพยาบาลด้วย
The present study was research and development which aimed at 1) analyzing the condition and needs for case management of postoperative breast cancer patients, 2) developing a pattern of case management, and 3) evaluating appropriateness the developed case management. The sample consisted of two groups by means of purposive sampling including criteria. The first group consisted of eighteen subjects ; one surgeon who was the head of the surgical unit, one surgeon, one assistant to head nurse, two head of the wards, seven professional nurses, and six postoperative breast cancer patients with family to analyzed the condition and needs for case management. The second group consisted of eleven subjects; ten representative sample of the first group, and one expert to evaluated appropriateness the developed case management. The instruments used in this study were composed of 1) conversation issues for analysis of condition and needs for the development of case management for service providers and service receivers, and 2) an evaluation appropriateness the condition of the developed case management. The instruments were validated to ensure content validity by a panel of four experts, with the content validity index equal to 0.88 and 0.83, respectively. Data were collected by individual and group of conversation, for content analysis, and calculation of appropriateness scores used were means and percentage. The findings were as follows: 1). The pattern of developed case management there are three components; (1) structure, has devised a policy, goal, the specification of work assignment and standard practice, (2) process, the care provisions on case management for implementation and continuously monitor evaluation, (3) outcome, the service receivers received the quality services, respond to the service receivers’ needs and satisfaction of services. 2). Evaluating appropriateness the pattern of the developed case management, the findings of the appropriateness were means scores percentage to 91.16. As regards the recommendations for hospitals that wish to implement the developed case management, the needs of the hospital staffs and the suitability to the contexts of the hospital need to be taken into consideration as well.
