Abstract
การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของการจัดโปรแกรมบริการพยาบาลที่เสริมสร้างความรู้และความสามารถในการบริหารมือด้วยตนเองต่อความรุนแรงของอาการและความสามารถในการใช้มือของผู้ป่วยกลุ่มอาการอุโมงค์ข้อมือที่บำบัดแบบผู้ป่วยนอก กลุ่มตัวอย่าง ได้แก่ ผู้ป่วยที่ได้รับการวินิจฉัยว่าเป็นโรคกลุ่มอาการอุโมงค์ข้อมือที่มารับบริการที่ต้องตรวจศัลยกรรมกระดูกและข้อ แผนกผู้ป่วยนอก โรงพยาบาลสุราษฎร์ธานี จำนวน 40 ราย แบ่งเป็น 2 กลุ่ม คือ กลุ่มที่ได้รับบริการตามกิจวัตร และกลุ่มที่ได้รับบริการพยาบาลตามโปรแกรม กลุ่มละ 20 ราย เครื่องมือในการวิจัย ได้แก่ 1) โปรแกรมบริการพยาบาลที่เสริมสรัางความรู้และความสามารถในการบริหารมือด้วยตนเองสำหรับผู้ป่วยกลุ่มอุโมงค์ข้อมือที่ผู้วิจัยสร้างขึ้น 2) แบบสอบถามข้อมูลทั่วไป 3) แบบสอบถามบอสตัน ฉบับภาษาไทยของ สิทธิพงษ์ อุปถัมค์ และ วิลู กำเหนิดดี (2008) ซึ่งประกอบด้วย ส่วนที่ 1 ประเมินความรุนแรงของอาการ จำนวน 11 ข้อ และ ส่วนที่ 2 ประเมินความสามารถในการใช้มือ จำนวน 8 ข้อ 4) แบบประเมินความเจ็บปวดแบบตัวเลข 1-10 และ 5) แบบประเมินความพึงพอใจในโปรแกรมที่ได้รับที่ผู้วิจัยสร้างขึ้น จำนวน 15 ข้อ เครื่องมือทั้งหมดผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ จำนวน 5 ท่าน ความเที่ยงของแบบสอบถามบอสตัน ในส่วนของแบบวัดความรุนแรงของอาการ เท่ากับ 0.90 และความสามารถในการใช้มือ เท่ากับ 0.95 และความเที่ยงของแบบประเมินความพึงพอใจ เท่ากับ 0.80 วิเคราะห์ข้อมูลโดยการใช้สถิติ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่ามัธยฐาน ส่วนเบี่ยงเบนควอไทล์ เปรียบเทียบความแตกต่างของของสองกลุ่มด้วยสถิติ Mann-Whitney U Test
ผลการวิจัยพบว่าผู้ป่วยที่ได้รับบริการตามโปรแกรมมีความรุนแรงของอาการลดลงมีความสามารถในการใช้มือเพิ่มขึ้น และมีความเจ็บปวดน้อยกว่าผู้ป่วยที่ได้รับบริการตามกิจวัตรอย่างมีนัยสำคัญทางสถิติ (p < 0.001) และ มีความพึงพอใจในโปรแกรมในระดับมาก (M = 3.94)
The purposes of this quasi-experiment research were: to study the effects of a nursing service program enhancing knowledge and ability of self-hand exercises on severity of symptoms and ability of hand functions of patients with carpal tunnel syndrome treated at the Out - patient Unit. The sample in this study included 40 patients diagnosed carpal tunnel syndrome treated al the Out-patient unit. They were divided into two groups equally: a control group was received routine care; whereas, an experimental group was received care according to the nursing service program. The research instruments consisted of: (1) the nursing service program enhancing knowledge and ability of self-hand exercises for patients with carpal tunnel syndrome, (2) the general data form,, (3) the Thai version of Boston questionnaires which were translated by Sitthipong Upatham and Wipoo Kumnerddee (2008), (Boston questionnaires consisted of two parts: part 1 evaluation of symptoms severity (11 items) and part 2 evaluation of ability of hand functions(8 items)], (4) Numerical rating pain score (1-10), and (5) Patients’ satisfaction scale (15 items). Content validity of all instruments was verified by five experts. Cronbach’s alpha reliability coefficients of Thai version for part 1 and part 2 of Boston questionnaires were 0.90 and 0.95 respectively and of patients’ satisfaction scale was 0.80. Data were analyzed by percentage, mean. standard deviation, median and inter-quartile, and Mann-Whitney Test. The results showed that the patients in the experimental group significantly decreased symptom severity, increased functional status, and decreased pain score more than the patients in the control group (p < 0.001). The patients in the experimental group rated their satisfaction at the high level (M = 3.94).
